Criswell & Criswell Plastic Surgery offers a secure login to our electronic medical record so that you may complete your new patient forms in the convenience and privacy of your home. The Patient Portal saves you time before your appointment, and is a safe method of keeping your information current at our office. And, it allows Criswell & Criswell Plastic Surgery to Go Green by saving paper!

Just click on the computer icon below, and you will be directed to a new window where you will be asked for your security code. Please call our office and we will email the security code to you. Then, you will be able to create your own username and password.

Step 2: Sign Welcome Letter & Consent Forms

Through your Patient Portal, please review the Welcome Letter & Consent Forms and sign forms.

The welcome letter is packet of information that is meant to inform you of the cosmetic services, both surgical and non-surgical, offered at Criswell & Criswell Plastic Surgery. It will also contain information about our locations, the appointment and treatment process, as well as general financial information about our practice.

The new patient consent forms allow for safe, comprehensive treatment. Our staff will be with you all the way: sharing our surgical skills and genuine concern about you and your experience. For a successful outcome, we will need to form a mutually responsible partnership. You will need to read and understand these consents.
